Case Manager BHH PT - 20 hours
Location: 610 Valley Health Plaza, Paramus, NJ 07652
On-site Part-time 20 hours:Program runs Between hours of M-F 8:00 am - 4:00 pm.
Hourly: $20.50

Job Summary:
As a Case Manager in our Behavioral Health Home, you will have the opportunity to work with adults in Bergen County with serious mental illness and chronic medical conditions, with the goal of improving their physical wellbeing through integrated care coordination through the use of various evidence-based modalities (such as CBT, MI, etc.).
