Are you an experienced Care Partner or Case Manager looking for a varied role where you can make a meaningful difference?
Resthaven is seeking two Care Partner - Relievers to join our Community Services team, with one position based at our Marion and one position based at Noarlunga.
As a Care Partner – Reliever, you will work across our Support at Home program, providing additional support across care portfolios to ensure continuity of high-quality, person-centred care. You will oversee and monitor services, review outcomes against individual care plans, and partner with older people, their families and the broader care team to support clients to remain living safely and independently in their own homes.
