The Care Management Support Assistant 2 contributes to administration of care management. You will provide non-clinical support to the assessment and evaluation of members' needs and requirements to achieve and/or maintain optimal wellness.
The Care Management Support Assistant will guide members/families toward, and facilitate, interaction with community resources appropriate for the care and wellbeing of members such as housing, food stamps and other community assistance available.

**Required Travel to Different Locations:**
**Conviva Little Havana - 2 days a week**
**Conviva Plaza Linda (Westchester)- 2 days a week**
**Conviva East Westchester - 1 day a week**
