Job Scope Programme Development and Implementation

• Assist in running targeted outreach and cognitive stimulation activities.
• Support in initial assessment of walk‑ins and referral cares, follow‑up intervention plans for clients as per the prescribed care plan and under the guidance of Allied Health Professional.
• Support in monitoring the client's conditions and progress and escalating to Allied Health Professional where necessary.
• Plan and conduct outreach to the community and stakeholders to promote mental wellness and mental health awareness, including community mapping.
• Assist in executing structured preventive activities and/or programmes for the community, those at risk of mental health conditions and clients.
