Our Amplar Preventative Health team helps Australians improve their health, prevent avoidable hospital admissions and achieve better long-term outcomes.

We have an exciting opportunity for a Cardiac Rehab Program Clinician to join our national Heart Health at Home program.

Using your cardiac rehabilitation expertise, you'll support participants recently discharged from hospital following a cardiac event. Managing your own caseload, you'll deliver evidence-based coaching, education and care planning via phone/video, helping participants build confidence, improve heart health and reduce the risk of future events.

Key responsibilities include:

Delivering virtual cardiac rehabilitation coaching and education.

Conducting assessments and developing personalised care plans.

Monitoring participant progress and managing clinical risks.
