Bereavement Coordinator, Inpatient Hospice
Part time
8 a.m. to 4:30 p.m.
Job Summary
Develops and implements the bereavement program. Oversees bereavement plans for each patient and family. Provides group support and individual counseling to bereaved clients per bereavement plan of care.
Essential Duties
+ Assesses bereavement needs and provides bereavement follow-up to each bereaved referred to the Bereavement Program.
+ Plans, develops, and implements Bereavement Support Groups and educational workshops on specific grief issues. Plans Memorial Service(s). Participates in Team meetings.
+ Participates in in-service, staff and other agency meetings for the purpose of planning, coordinating and generally improving the quality of service provided by the agency.
