Job Description
Become a Case Manager in South Simcoe with CMHA-YRSS, where you'll provide mental health support to clients. This full-time role focuses on recovery and community integration.
As a key member of the Assertive Community Treatment Team, you'll work with adults facing serious mental health challenges. Your role involves identifying client strengths, setting goals, and connecting individuals to vital resources. You'll deliver personalized services while promoting independence and recovery within the community.
Key Responsibilities:
• Maintain a caseload following Ontario's health standards
• Develop individualized empowerment plans for clients
• Respond to client needs with effective listening
• Facilitate referrals to other programs or agencies
• Collaborate non-judgmentally with clients on goal setting
Requirements:
• Degree or diploma in health or social services
• Registered with the OCSWSSW or equivalent
• Experience with adults in community settings
