Be an integral part of Baycrest's Ambulatory Services as a Registered Nurse (RN)! Focus on health navigation and chronic care for seniors across Toronto.
In this role, you will collaborate with the Interprofessional Primary Care Team, providing nursing clinics and support for clients living independently. Emphasizing wellness and chronic disease management, you will enhance the quality of life for senior clients through careful assessments and coordinated care with physicians and community partners.
Key Responsibilities:
• Deliver comprehensive health care and support
• Run nursing clinics and conduct community health visits
• Develop educational programs for health promotion
