**Advanced Practice Provider for Cardiology Heart Failure Program – Wheat Ridge, CO**

**About this role:**
Intermountain Health is seeking an experienced Nurse Practitioner or Physician Assistant for our Heart Failure Clinic. The selected candidate will partner with the a certified Heart Failure Cardiologist to continue to build a pre-eminent Heart Failure Program in the state of Colorado.
+ You would practice at our Wheat Ridge Cardiology clinic and Lutheran Hospital (https://intermountainhealthcare.org/locations/intermountain-health-lutheran-hospital) . Additional locations may be added with program growth
+ Hours are 4/10’s Mon-Fri, no call and no weekends. Your day off will vary
