Wage
$48.210 - $64.500 per hour
Responsibilities
- Meet medication related needs of patients, including resolving and preventing drug‑related problems.
- Work with patients and the health‑care team to provide direct patient care.
- Participate in patient education.
- Facilitate medication reconciliation activities.
- Teach students.
- Provide education to nursing and medical staff.
- Support the drug distribution system.
Qualifications
- Current registration in good standing with the Ontario College of Pharmacists in Part A of the register.
- Practical knowledge of pharmacology, current best practice clinical guidelines, and ability to interpret lab values....
